Data breach incidents are very expensive for organization, both in hard dollars and reputation costs.  To better understand the factors leading to a breach the Society of Corporate Compliance and Ethics and the Health Care Compliance association jointly fielded a survey to determine the prevalence, causes and consequences of data breaches.   The survey revealed persistent problems fueled by simple, human error.
